Children's Bridge Program (C.A.R.E.S.)

The Children's Advanced Relief, Encouragement and Support (C.A.R.E.S.) program provides palliative care services to children in the comfort of their home.

From a patient's initial diagnosis and throughout his or her illness, our interdisciplinary team, consisting of a pediatric associate medical director; pediatric navigator; child life specialist; nurses; social workers; physical, occupational and speech therapists; chaplains; and volunteers work together to ensure patients and their families receive the physical, emotional, social, psychological and spiritual support they need.

C.A.R.E.S. services include:

  • Expert management of pain and other symptoms by the interdisciplinary team, including support from Carroll Hospice nurses
  • Emotional and spiritual support for the patient and the family
  • A pediatric navigator to coordinate communication to make sure a patient's treatment is aligned among all care providers
  • Child life specialist to provide education and support to the patient and loved ones to ensure everyone understands the illness, treatment plan and goals of the patient, family and interdiscipliary team
  • Social workers to counsel and provide support with the legal, financial and social issues that come with a terminal diagnosis
  • Seamless transition to hospice care when the time is appropriate
  • Bereavement counselors to offer grief counseling after the loss of the child
