
The Carroll Home Care Bridge Program blends Carroll Hospice and Carroll Home Care services based on the patient's needs. The Bridge patient is designated by a physician as hospice appropriate and meets the eligibility criteria for hospice admission.
The Bridge Program offers palliative care and aggressive symptom management with the patient’s health care goals being paramount. Patients of the Bridge Program may be actively involved in curative therapies, and need support and symptom management to successfully complete a treatment regime.
Bridge patients with Medicare benefits will follow the Medicare Home Health Guidelines of needing skilled nursing care and being homebound. The Bridge Program's plan of care integrates psychological and spiritual aspects, as well as a support system to help families cope with the patient’s illness.
